Help with a gift...

I have a couple of questions regarding our class buying our Sensei a gift for his wonderful work instructing us.

First, we are considering getting him a new Hakama with his name embroidered on it.

My questions are:

1) Is there any reason this gift would not be proper etiquette for a class to get their Sensei?

2) For the name, should we use his first name, first and last, or last name?

3) Is there a ceremony or at least, a proper way of presenting it to him?

I appreciate your help in this matter.

I never see anything wrong with giving a gift for your sensei.

Use last name, the family name.

No proper ceremony. It's not proper to interrupt class time, so we usually give it by the end of class.

Surprise your sensei.

When I have to die by the sword, I will do so with honor.
