Any examples of Dojo surveys?

I am looking for a good example of a general survey used by any dojos out there to gauge the quality of their services....to include quality of instruction, convenience of schedule ....etc.

Any help would be greatly appreciated.

I'd think that the questions you ask should be based on the information you want to know. Using somebody else's survey might not give you that. Common sense should be your guide.

Clark, I agree with you...but I would also imagine that there would be a lot of similar questions from dojo to dojo....plus a well designed survey is a valuable tool.
